Build a form and review its submissions
Build a registration or application form, publish it, collect fees and signatures, and move every submission through review.
Forms is for anything you’d otherwise collect on paper or by email: team and player registrations, tryout applications, waivers, event sign-ups. Your registrar builds the form, your reviewers work through submissions with a full status history, and applicants get an email at every step.
Build the form
- 1Start a form
Go to Studio → Modules → Forms → All Forms and create a form. Start blank or from a template: Contact us, Team registration, Player registration or Event registration.
- 2Drag in fields
Drag fields from the palette onto the canvas and into order. Click a field to set its label, help text and whether it’s required; click it again, or press Escape, to deselect it.
- 3Split long forms into pages
Drop a Page break wherever a new step should start. Applicants move page by page and can save a draft to finish later.
- 4Show a field only when it matters
Turn on Only show this sometimes on a field and pick the earlier answers it depends on. A hidden field is never required and its answer isn’t saved.

Set it up and publish

- 1Publish
Set the form to Published. Its other states are Draft, Paused and Archived.
- 2Share the link
Press Copy public link and share it, or turn on Show in website footer. Unpublished forms never show in the footer.
Charge a fee
- 1Switch the fee on
In the form’s settings, turn on Charge a fee for this form and fill in Amount, Payment title and Payment details shown to the applicant.
- 2Choose one-off or monthly
Leave Charge this every month off for a registration or entry fee. Turn it on to bill the applicant’s card every month until it’s cancelled; monthly fees need Stripe.
- 3Offer a discount code
Optionally, create a code in Studio → Management → Coupons and apply it to every paid form or to the forms you choose. Codes work on one-off fees only.
Payment is always the last step: the applicant answers everything, then pays. Until the payment succeeds the submission sits as Awaiting payment and nobody is notified. A code that covers the whole fee skips the payment page.
Review submissions

- 1Open Responses
Go to Studio → Modules → Forms → Responses. Filter by form or status, or search by applicant.
- 2Open a submission
Read the answers, open uploaded files and see the drawn signature.
- 3Change the status
Pick the new status and add a message for the applicant if you need to. They get an automatic email.
- 4Keep notes internal
Write anything only your team should read in Internal note (never shown to the applicant).
Select several submissions to change their status together. Export CSV downloads the answers and Documents (ZIP) downloads the uploaded files. Applicants follow their own progress from a private View application status link.
